Here you'll find the release forms and statements to which every trip participant must agree before going on the mission field with NEXT. Simply click on the links below to access web and PDF versions of each form. Adult Trip Release Form - a legal form which must be signed and notarized outlining the terms of your agreement to participate in a NEXT trip. Minor Trip Release Form - a legal form which must be signed and notarized by both parents/guardians outlining the terms of your student's agreement to participate in a NEXT trip. Conduct Statement - a list outlining conduct expectations for each participant on a NEXT trip. Belief Statement - a doctrinal statement of belief that outlines broad theological points to which every NEXT trip participant must agree and uphold.
